Leviathan Campaign Supplement Forms

The Leviathan Campaign Supplement Forms can be used to aid in Leviathan planning missions, campaigns, and all the forces necessary to execute them. There are eight PDF forms, with seven of them being formatted to be fillable for electronic use or filling and then printing.

Battle Group (Fillable)

This form provides space for battle groups of up to 16 ships, 20 fighter groups, and 5 other assets attached to the group. Additionally, there is space to record mission-specific details for the battle group.

Campaign (Fillable)

Record all the details for a campaign on this form which also provides a handy flowchart of the campaign round and several campaign-specific tables for quick reference.

Captain’s Log (Fillable)

Record, advance, and track the careers of your favorite captains using this form.

Fighter Group Record (Fillable)

Record four squadrons of fighter groups on this form which replicates the fields on the fighter group cards for tracking their status during game play. This is a must for games with several carriers.

Fortifications (Reference)

This is a reference form for the six base fortifications for quick use in game play.

Mission Briefing (Fillable)

The Mission Briefing form covers all the details of the mission system and the selected parameters for game play as a reference. This form can be used to record each step of mission generation and save the details of a mission if it cannot be completed in one day.

Settlement (Fillable)

Build your own custom settlement using this form. Settlement reference charts and a summary of settlements creation are conveniently located on the form.

Task Force (Fillable)

Create and record your own task force and track its progress throughout campaigns and missions. This form can be used to record pre-designed task forces for quick battle group selection or in campaigns that track the status of each ship in the task force as they return from their missions.
